Requirements to Participate in Athletics at Lin-Wood
Each & every school year (prior to participation) - Students in Grades 6-12 wishing to participate in extra-curricular activities must:
Have an updated Medical Release Form completed and turned into the Middle/High School Main Office. Medical Insurance coverage information should be included.
Provide the office with a written medical statement from their physician (within the meaning of NH RSA 329) certifying the student athlete has passed an annual sports physical examination.
